Brain Injury (BI)

The Brain Injury (BI) waiver is a habilitative/rehabilitation and independent living program emphasizing developing new independent living skills and/or re-learning lost independent living skills due to an acquired or traumatic brain injury. Participants who have a medically diagnosed brain injury receive intensive therapies and services based on the goals of the participant, their providers, and their Managed Care Organization (MCO). The participant's progress will be evaluated every six months until they no longer require these services or reach a plateau. At that point, they can transition to another waiver for ongoing supports.

Brain injuries diagnosed as being a chromosomal or congenital condition do not qualify for the BI waiver. Suppose a brain injury is obtained before the age of 22. The individual may be considered developmentally disabled and referred to the Community Developmental Disability Organizations (CDDOs) before BI screening. Money Follows the Person Program

Money Follows the Person (MFP) is a federal Medicaid demonstration program that helps states transition older adults and individuals with disabilities from nursing homes and other institutions into qualified community-based residences.

How Money Follows the Person Works

  • No Direct Cash: Beneficiaries do not receive direct money; instead, the program provides federal grant funding to states.
  • Transition Services: States use these funds to pay for transition costs, home-and-community-based services (HCBS), furniture and household goods/necessities needed in order to move back into the community.
  • 365-Day Support: MFP enhanced federal matching funds and specialized support last for the first 365 days (one year) after a person leaves an institution. After that, ongoing care shifts to standard Medicaid HCBS waivers if the individual remains eligible.

Eligibility and Qualified Settings

  • Institutional Stay: Candidates generally must reside in a hospital, nursing facility, or intermediate care facility for a required consecutive period (typically 90 or more days).
  • Qualified Residences: Participants must move into a home-like setting, such as an individual house, an apartment, or a group home with a small number of unrelated residents (usually four or fewer).
